Studies have found that the transitional care intervention (TCI) can reduce readmission rates in different populations. Evidence shows that it can be applied in patients with chronic illnesses, such as cardiac transplant patients. Transitional care intervention is practical and useful in the clinical arena and can be implemented by an APRN using simple steps. TCI implementation also fosters interprofessional collaboration by relaying information to PCPs and specialists in a timely manner. In addition, the study demonstrated the value of APRN-led intervention as a means of providing high-quality and cost-effective care to reduce readmission rates in the cardiac transplant populations.
