A community hospital has developed a standardized fall prevention communication tool that improves its fall prevention culture for safety. The tool reduced the fall rate for both medical-renal unit and oncology unit to signify the effectiveness of the standardized practice. The best-practices in preventing falls in the hospital are also presented.
