An emergency medical services response team caring for a patient who'd suffered a cardiac arrest accidentally began infusing a 1,000 mL bag of sterile water for inhalation instead of an IV solution. Given in substantial quantities, hypotonic sterile water solution can cause hemolysis and lead to serious harm, including death. Fortunately the ED staff caught the error and quickly replaced the solution with 0.9% sodium chloride for injection.
