Adverse drug events (ADE), defined as expected or unexpected injury resulting from the use of a drug,1 remain among the most frequently occurring adverse events in hospitalized patients with estimates ranging from 380,000 to 450,000 preventable ADEs each year.2 ADEs include injury caused by the drug (adverse drug reaction, overdose) and injury from the use of the drug.1 There are several origins or types of ADEs: medication errors (e.g., administration of wrong drug, overdose), adverse drug reactions, therapeutic failures, and withdrawal.
