N2  - The collection of subjective and objective data relating to a patient's current and past medical history is common practice for many health care professionals. The approach and format may be different depending on the individual and their discipline and in terms of the documentation, yet the overall task is fairly routine. The use of technology and electronic health records have influenced this process to some extent, as has the need for standardized language to demonstrate outcomes. Although all of this specific information and appropriate documentation is vital and required in many cases for the patient's plan of care, the way in which it is obtained can be extraordinary.
